| Medications:
No student may possess, administer or distribute prescription or non-prescription medications while in the School, or at any School sponsored activity.
All medications (prescription or over-the-counter) must be administered in the Health Room under the supervision of the school nurse or her designee.
The parent/guardian must administer the first dose of any new prescription or over-the -counter medication.

| Prescription Medications:
Before a prescription medication will be given during school hours the following directives must be followed:
A person authorized to prescribe in the State of Maryland (physician, physician's assistant, nurse practitioner or dentist) must complete and sign a medication order form. This form must be given to the School Nurse, or her designee, along with the medication. Medication order forms are available from the Health Room.
Written parental consent is required for each medication ordered and for each new medication ordered by the healthcare provider.
The original prescription container must accompany all medications to be given in school. Parents should request two containers from the Pharmacist, one for home and one for school. Medication must be brought to school by the parent/guardian. If the parent is unable to do so please telephone the School that the child is bringing the medication to school and how much medication is being sent.

| Over-the-Counter (OTC) Medications:
OTC medications including creams, eye drops, and nasal sprays must be brought to the Health Room and taken in the presence of an adult.
OTC drugs require a written order from the student's primary medical care provider.
The first dose of all OTC medication must be given by the parent/guardian.
All OTC medications must be in the original store container.

| Controlled Substances (i.e. Ritalin, Narcotic pain medication, etc):
The parent/guardian must bring the medication to School.
No more than a 30-day supply of the controlled medication should be sent to School.

| At the Kindergarten Campus All prescription and over the counter medications should be given to the child's teacher. The teacher will make arrangement to administer the medication as outlined above. |

| Medical Records Every student entering St. Pius X Regional School must comply with the Prince George's County Health Department immunization requirements. You must give the School a signed letter, official shot record, or other signed form from your doctor or local Health Department, which clearly shows the dates on which all the required immunizations were administered.
All student immunization records must be accurate and current. The Prince George's Health Department inspects student medical records and will hold the parent responsible for compliance with their regulations. Therefore, all student records must be complete and up-to-date within the first two weeks of school. If the school nurse notifies you that your child's records are incomplete you must handle the matter immediately. |

| Health Screening Vision and hearing screening will be administered to students in grades 1, 3, 5, and 7 each year, also to any new students in grades other than those listed above.
Scoliosis screening will be conducted for all eigth grade boys and sixth grade girls. |
